If you’d like to order the Syn-One Test® or Small-Fiber Dx®, but prefer not to perform the biopsy procedure yourself, the following information will help to ensure a smooth referral process.

Frequently Asked Questions

Can I order the test for my patients even if I prefer not to perform the biopsy myself?

Yes, CND can help train another provider on the biopsy procedure, and you may refer your patients to them for all biopsies.

I don’t currently have a clinician in mind to do the biopsy. Do you have any clinicians I can refer my patients to?

We can identify dermatologists, home health groups, or other clinicians in your area who can perform the biopsy on your behalf. Please contact your clinical account manager or sales representative for assistance.

How can I make sure the results are sent to me?

When completing the paperwork, be sure to list your name and practice information as the ordering clinician. The clinician who is performing the biopsy should be listed as the performing clinician. This ensures CND sends the test results directly to you as the ordering clinician.

What should I give to the clinician who will be performing the biopsy?

Please provide the completed requisition form, patient insurance (front/back), demographics, and chart note to performing clinician.

We have outlined the general process below, but your sales executive and clinical account manager can help you establish a workflow that best fits your practice.

What do I need to provide to the performing clinician?

Complete all required CND paperwork before sending the referral. Ensuring accuracy helps our team resolve any discrepancies and deliver the final pathology report to the correct practice.

   Include the following information with your referral:

  • Completed test requisition form (fill out all sections except the biopsy-specific information, performing clinician details, and final signature)
  • Patient insurance information (front and back)
  • Patient demographics
  • Office chart note
The performing clinicianwill complete the biopsy procedure, finalize the paperwork and send all required materials and specimens to CND’s lab. CND will send the final report to your practice approximately three weeks later (timelines may vary).

For Clinicians: Request a Skin Biopsy Test Kit

Our suite of neurodiagnostic tests provides clinicians with objective pathological insights on diseases that are often difficult to diagnose. Using our Skin Biopsy Test Kit, collect three small skin biopsies from your patient, send them to our CLIA-certified and CAP-accredited lab, and CND will provide important pathological insights to support a diagnosis.
